Eli Lilly formed Lilly ICOS LLC in 1998, the same year Viagra proved someone would pay for an oral PDE5 tablet. ICOS kept chemistry and early development. Clinicians should know the 5 mg PRN step was not an in-house Lilly synthesis from day one. It was a PK find a large shop finished.

Brock assigned 151 men to 5 mg before sex, not before toast
Gerald Brock's integrated analysis (Journal of Urology, October 2002) is the public face of that matrix. Mean age 59. Mild to severe ED, mixed etiologies. Co-primaries: IIEF erectile-function domain, SEP2 (penetration), SEP3 (completed intercourse). A Global Assessment Question sat as extra. Food and alcohol were left unrestricted in the protocol language - a quiet contrast with the sildenafil plate lecture men already knew.
On 5 mg as needed, mean IIEF-EF rose 4.6 points versus 0.6 on placebo. SEP2 success improved 16 percentage points. SEP3 improved 22. GAQ improved-erection rate sat at 50 percent versus 35 percent on placebo. Those are PRN numbers. They sold a smaller night tablet for men who did not need, or did not tolerate, 10 or 20 mg. They did not sell a 30-day habit.
The 20 mg cell is the one ads later loved: +7.9 IIEF-EF, about 75 percent of attempts completed, 81 percent saying erections improved. Fine. Quote it as 20 mg. Porst's timed 36-hour work used 20 mg, not 5. Europe cleared tadalafil in late 2002. FDA followed on 21 November 2003. The 5 mg strength existed in the trial matrix as a night chip. It was not the launch identity, and it was not a daily identity.

Three oral PDE5 inhibitors shared one US calendar year: vardenafil earlier in 2003, tadalafil in November. Differentiation had to be immediate. Lilly's pitch was pharmacokinetic. Why would it? Daily dosing looks like a compliance product. PRN looks like a night out. Marketing a long window is easier than asking a man to take a tablet on Tuesday when nobody is home. The science for daily was already whispering - 17.5-hour half-life, accumulation to a usable trough - but whispering is not a labeled row.
Renal and hepatic footnotes on the first label already used once-daily language for damaged clearance, which confuses readers later. That is a safety cap for people who cannot clear a PRN bolus, not the 2008 once-daily ED indication. Do not collapse those sentences. FDA approved tadalafil for signs and symptoms of BPH, and for men who have both ED and BPH, on 6 October 2011. The dose for those urinary indications is 5 mg once daily. It is not a secret daily. Mix those contracts and men skip the night tablet because they think they already took a breakfast dose last month - or they swallow a morning almond and call it PRN.
