Absolute bioavailability is about 15 percent. Fasted Tmax after 20 mg sits between 30 minutes and 2 hours, median 60 minutes. Half-life is about 4 to 5 hours. Same-evening drug. Dose about 60 minutes ahead. This is not tadalafil's long tail and it is not a next-morning cover.
High-fat meals are the PK hold I teach before the first fill. Cmax fell 18 to 50 percent across the food-effect work. Some men feel a late, weaker peak. Some feel nothing useful. The Wuppertal origin note is the chemistry and the launch. Food still matters. High-fat meals cut film-coated Cmax by 18 to 50 percent in the two food-effect studies. Staxyn is also food-sensitive. I still write 'light stomach, about an hour' on the instruction line. The dissolve-on-the-tongue trick does not cancel dinner.
Fifteen percent bioavailability and a four-to-five-hour tail

Age and liver change the start. Sixty-five and over: consider 5 mg first. Child-Pugh B: start 5 mg, max 10 mg. Those are labeled moves, not coupon tricks. A 20 mg tablet in that group is a later step, not a first reflex.

Nitrates remain an absolute contraindication - nitroglycerin, isosorbide, poppers, any nitric-oxide donor. Same 24-hour gap after a dose. Same duty to tell EMS. This is not the distinguishing hold. The distinguishing hold is the antiarrhythmic list.
Avoid vardenafil with Class IA (quinidine, procainamide) and Class III (amiodarone, sotalol) agents, and in congenital long-QT.
